Five things a strong safety answer has to show

A correct answer isn't enough on its own. It has to be in the right order, specific to the site, and clear about where the person's own authority ends.

Why one right answer isn't enough

Multiple-choice checks tell you whether someone can recognise the right option. Real incidents don't come with options. A colleague who knows the first step but not who to tell, or who logs nothing afterwards, can still leave a gap that matters.

So instead of marking an answer right or wrong, it helps to look for the parts a safe response needs. In workplace settings we look for five.

  • ActThe correct first action
  • WhyTied to this site's risks
  • EscalateWho to tell, and when
  • RecordWhat to log, and where
  • BoundaryWhat not to do themselves

What each marker means

Act

The correct first action for this situation, in the right order. Making the area safe usually comes before reporting it.

Why

The reason, tied to this site. “Because it's a fire door on an escape route” shows more understanding than “because it's the rule”.

Escalate

Who to tell, and when. A strong answer names the role and the threshold, not just “my manager”.

Record

What gets written down, and where. This is the marker most often missed, and the one auditors ask about first.

Boundary

What the person should not do themselves. Knowing where your authority ends is part of competence.

Turning markers into a decision

Markers help a reviewer see what's there and what's missing. They don't make the decision. In VeriViva, each question sets out what a good answer covers and what the person would not do, and the reviewer records whether each answer met it, mostly met it, partly met it or didn't evidence it. A named person is always responsible for the overall Viva decision: evidenced, or follow-up needed.

Why AI should never be the one who decides

VeriViva prepares the evidence. A named person makes the call. Here's why we hold that line, and what it means in practice.
